This physician will lead the client's site-based clinical services, including strategic oversight and operational performance. Additionally, this physician will both perform and supervise clinical aspects of each clinical component under PCMH to include but not limited to primary care, executive health, sports medicine, psychiatry, and preventive partnership program.
Administrative:
- Aligns PCMH clinical services with client company's mission, priorities, and values
- Establishes the PCMH operating system
- Assures efficient, effective, and empathetic delivery of care
- Assures that various lines of clinical services are fully integrated
- Develop a patient centered medical home outpatient primary care practice at an assigned employer-based site as directed by Atlantic Corporate Health and Hospital's Chair, Department of Internal Medicine, or his designee.
- Works in conjunction with corporation representative focusing on achieving patient centered medical home criteria
- Provides proactive outreach to complex high-risk patients to create and manage their plan of care including, disease education, and coordination of services
- Creates and implements population management strategies that support patient self-engagement to manage chronic diseases
- Participates in special projects, metrics management, NCQA submissions, monthly and quarterly reporting, and cross-training with other members on the PCMH team.
- Consistently applies the principles of comprehensive, community based, patient-centered, developmentally appropriate, and culturally and linguistically appropriate care coordination
- Provides program consultation, administrative and technical assistance to corporate benefits personnel
- Monitors employee program outcomes to create compliance and improve health outcomes
- Provides overviews, updates, statistics of such program at regular intervals
- Acts as a liaison for on-site clinicians in the delivery of corporate services including employer-based fitness center programming, health fairs, satellite and live lunch and learn programs and screenings
- Measures, analyzes, and reports key performance metrics as directed by the client
Clinical:

Provide clinical oversight of all clinical practitioners under the PCMH to
include but not limited
to primary care, executive health, sports medicine, psychiatry, and preventive partnership
program. -
Provide appropriate primary care services within the scope of the
Physician's license, including but not limited to: Format change
- Obtains complete history and record findings in a systematic format
- Performs complete physical assessments
- Orders, performs, and interprets routine laboratory tests
- Orders medications and or medical devices as needed

Follows hospital care and round or communicate with hospitalists on
the care of
hospitalized patients - Ensures that appropriate preventative medicine testing is conducted on a routine basis
- Participates with all quality improvement and assurance initiatives
- Conducts appropriate follow up to ensure appropriate levels of patient care
- Serves as site administrator for the clinic during emergencies
- Maintain the standard of care for the delivery of internal medicine
- Participate in after hours and weekend telephone coverage for the Division of Internal
- Medicine on an equitable rotational basis as scheduled by the Director.
